Recognition of non EU nurse qualifications in Italy
The Ministry of Health decides from 90 days, with a 16 euro stamp duty per qualification, and the decree holds 2 years for OPI registration.
A nurse holding a qualification earned outside the European Union may practise in Italy only after two separate acts: a recognition decree from the Ministry of Health and registration with the Ordine delle Professioni Infermieristiche, the nursing board. The Ministry states processing from 90 days once documentation is complete. The decree alone does not authorise practice.
The application to the Ministry of Health
Recognition of health professional qualifications is governed by Legislative Decree 206/2007, which transposes the European rules on the subject. The application goes to the Ministry of Health, to the directorate responsible for the health professions. The point that most often surprises employers is that the application is required even when the qualification has already been recognised in another EU member state: a Spanish or Portuguese decree on an Indian or Filipino qualification has no automatic effect in Italy.
The basic administrative cost is published: a 16 euro stamp duty for each qualification to be recognised. A nurse asking for recognition of two qualifications, a basic diploma and a specialisation for example, pays twice. That item is small against the rest of the file, which includes sworn translation and legalisation of the foreign qualifications, and those are the items that actually move the budget and the calendar.
The timeline the Ministry of Health indicates runs from 90 days once the documentation is complete. Complete is the operative word: the clock does not start when the application is filed, but when the file has no gaps left. A missing certificate of conformity or an unsworn translation moves the start of the period, not its length.
From decree to registration with the board
Once the recognition decree is issued, the nurse has two years to register with the territorial Ordine delle Professioni Infermieristiche. If that period passes without registration, the decree lapses and the procedure has to be run again from the start, stamp duty included. This is the deadline employers forget most often, because it falls when the file looks closed.
The risk is real where the decree arrives before the visa, or before the worker is physically in Italy. A decree issued in January 2026 for a worker who enters the country in 2027 leaves a narrow registration window, and OPI registration requires presence and residence documents. Sequence the two so the decree and the arrival do not sit far apart.

Entry outside the quota for nurses
On the immigration side, nurses hired by healthcare providers enter outside the quota under article 27, paragraph 1, letter r-bis of Legislative Decree 286/1998, the consolidated immigration act. The hire therefore does not compete with other applications for decreto flussi places and does not depend on the annual click day window. For a hospital or care provider that is the difference between planning on its own calendar and waiting for a quota.
The out of quota channel does not remove the professional requirement. The nulla osta rests on the applicant being a nurse, and in Italy that status exists only with the recognition decree and board registration. A provider that starts the entry file without having applied to the Ministry of Health ends up with a worker in Italy who cannot be employed as a nurse. A nursing recruitment that is set up properly holds three clocks together: the 90 days stated by the Ministry of Health, the two year life of the decree, and the out of quota entry date. Align them and the nurse works from the first available day. Treat them as separate files and the mismatch surfaces once the worker has already arrived.
