Registering a foreign worker for pension and health insurance in Croatia
Croatian employers register a foreign worker with HZMO on form eM 1P, no earlier than eight days before the first working day. HZZO health cover opens with it.
You register a foreign worker with the Croatian Pension Insurance Institute (HZMO) on form M 1P, or its electronic version eM 1P. The filing window opens eight days before the start of work and closes before the worker's first working day. Mandatory health insurance with HZZO opens through the same filing, with no second application.
Who files, and by when
The employer files, not the worker. The HZMO guidance on registrations and deregistrations sets two limits at once: the form cannot be submitted more than eight days before the first working day, and it must be submitted before the employment relationship begins. The insurance start date on the form has to match the start date written into the employment contract. When the two dates differ, HZMO returns the filing for correction, and in the meantime the worker is on site without valid cover.
HZMO also requires the employer to report the fact that insurance has begun within 24 hours. On a planned arrival the 24 hour rule and the "before work starts" rule collapse into the same day. They separate when the arrival date moves, which on a Nepal or India corridor it frequently does. The working rule is simple: the date on the form is the day the worker actually stood at the machine, and the filing goes in that same day.
Deregistration uses form M 2P, or eM 2P, on the same logic. The end date is the final day of the employment relationship as stated in the contract or the notice.
Two things must exist before the form can be opened at all: the worker's Croatian personal identification number (OIB) and an issued stay and work permit. Renewal dates and the fees attached to them are set out in the permit renewal deadlines and fees.
Paper form or the e prijave application
HZMO runs two channels. Employers with more than three insured persons are required to file registrations and deregistrations electronically, through the e prijave application. Smaller employers may choose. In practice any employer hiring foreign workers in cohorts crosses that threshold before the first group lands, so access to e prijave belongs in the preparation phase rather than in the paperwork that follows arrival.
Getting that access requires a digital certificate and a one time registration of the employer with HZMO, which takes several working days. An employer who leaves it to the week before arrival is left with the paper form and a counter, and the counter keeps its own opening hours rather than the start date in the contract.
Since 1 July 2025 the insurance basis for workers who are not Croatian citizens has been aligned with the basis for citizens. The visible consequence on the form is that eM 1P no longer carries a separate marker for a foreign national. Anyone working from guidance written before that change looks for a field that no longer exists and loses an afternoon hunting for a fault in the application.

Health cover opens with the same filing
This is where employers most often do work twice. Registration for pension insurance automatically triggers registration for mandatory health insurance, so no separate application goes to HZZO. HZZO picks the record up from the HZMO register through its e Zdravstveno system. What stays with the employer is the check that the status is genuinely open before the worker first needs a doctor, because a failed filing surfaces at the clinic desk and nowhere earlier.
Family members who arrive later follow a different route. Their cover as dependants of the insured person is filed with HZZO separately and does not open automatically with the worker's own registration. This is a standard second year omission, when a worker brings a spouse and assumes one filing covers both.
Neither HZMO nor HZZO charges a fee for submitting the form. The cost that arises is contributions. The same contribution rates apply to a foreign worker's salary as to a Croatian worker's, precisely because the basis was equalised in July 2025. An employer who budgeted for a lighter regime on foreign labour budgeted wrongly.
What an inspection compares
A labour inspection reads three documents side by side: the stay and work permit, the employment contract, and the insurance registration. All three have to carry the same employer, the same job, and the same start date. The expensive mismatch is the date. A worker who worked three days before the date on the registration worked three days undeclared in the record, whatever the contract signature date says. The employer side conditions that have to be satisfied before any of this begins are set out in the employer conditions for an HZZ positive opinion, and accommodation is inspected on its own standard, described in the worker accommodation standards and rent cap.
Insurance registration is the shortest step in the whole Croatian hiring sequence and the only one the employer completes without waiting on another institution's decision. That also makes it the only step where a delay has no external cause. If a cohort is arriving, clear the e prijave registration at the same time the permit application goes to the police administration, not on the day the flight lands.
